Sports Physicals

Physicals consist of two different parts, the physical and forms located on DragonFly Max. BOTH have to be completed in order to play a sport. For questions, please contact Mollie Jones at mollie.jones@putnam.k12.ga.us .

Part 1 is the section that the doctor completes. The accepted sports physical from the doctor is below:

Part 2 is DragonFly. You can upload your physical on DragonFly Max to be approved by the school or bring your physical to your coach to upload to your DragonFly account.
