MCV4 Vaccination Information

Below, you will find the English/Spanish versions of the parent letter about the new required MCV4 booster. All students 16 and older entering 11th grade MUST have this vaccine booster and present a complete copy of their Form 3231 for this school year. Parents can take their students to the Putnam County Health Department or primary MD to get the vaccine.
